Adds a report issue button + form that submits issues to Plain.
I also unified the existing feedback form's styling:
I updated the survey's copy in Posthog. Titles should always be short and to the point.
Event tracking in Posthog is included. Readme and all appropriate places are updated to include the newly introduced
